The Midwife will perform the following: · Obtain and record personal and family, medical, surgical and obstetric history. · Physical examination including gestational age, danger signs , breast examination · Risk grading · Laboratory tests (Ref to Maternal Health Record -HP-194 2015 (MHR), Pg-22) · Vaccination (if indicated) · Supplementation of folic acid · Health education and counseling for women (Ref MHR, Pg-15). · Refer to Doctor for risk assessment & viability scan · Schedule Follow up Appointment · GTT if indicated

General Assessment including BP, Wt, Fundal Height Routine laboratory- GTT, Urine GKP & M/E Risk grading Supplementation of T. Folic acid &T.Fefol x 2 months Counselon: Danger signs, diet and supplementation Schedule Follow up Appointment

• General Assessment including BP, Wt, Fundal Height Fetal Heart • Routine Urine GKP, GTT • Risk grading Supplementation of folic acid & iron • Health education and counseling for women (Ref MHR, Pg-15). • Refer to ANC Doctor (if indicated) • Schedule Follow up Appointment

• General Assessment including BP, Wt, Fundal Height, Fetal Heart, Fetal movement • Routine Urine GKP • Supplementation of folic acid & iron • Health education and counseling for women (Ref MHR, Pg-15), Kick Chart • Refer to ANC Doctor (if indicated) • Schedule Follow up Appointment

At Any Visit • Abnormal Assessment Findings • Presence of Danger Signs, abnormal investigation results • Past history of medical, surgical and gynecologic disease present. ↓ Referral to Doctor

General Assessment including BP, Wt, Fundal Height, Fetal Heart, Fetal movement ,Fetal presentation, Lie ,Level of Engagement, Danger Signs & Risk Grading • Routine Urine GKP • Blood Investigations Hb,ABS test (if indicated) • Supplementation of folic acid & iron • Health education and counseling for women (Ref MHR, Pg-15), Kick Chart • Reassessed by ANC Doctor if any Abnormal Assessment Findings , Presence of Danger Signs , abnormal investigation results • Appointment at 40 weeks if not yet delivered.
